
Case Manager - Residential
Provide comprehensive and integrated care for individuals facing substance use disorders and support clients on their journey towards recovery and improved quality of life. This role involves facilitating group sessions, connecting clients to critical resources, supporting life skills development, and creating tailored aftercare plans to ensure clients have the support they need upon discharge. The ideal candidate is passionate about recovery, skilled at building supportive relationships, and committed to fostering client empowerment and independence.
- Lead and co-facilitate group sessions on topics such as recovery skills, relapse prevention, goal setting, and personal empowerment. Foster a positive, engaging environment to encourage active client participation.
- Work closely with clients to create individualized aftercare plans that address their unique needs and support long-term recovery.
- Provide case management services, including linking clients to community resources, such as housing, employment assistance, healthcare, and legal services.
- Advocate for clients' rights and needs within the treatment system and community, including attending court hearings or meetings with social service agencies as needed.
- Monitor clients' progress in treatment and regularly review and update treatment plans to ensure they remain relevant and effective.
- Coach clients on essential life skills, including budgeting, time management, healthy lifestyle choices, and problem-solving strategies.
- Maintain accurate and up-to-date documentation of client interactions, services provided, and progress towards treatment goals.
- Encourage clients to develop supportive relationships and build a recovery-focused support network. Organize and participate in community connection activities and skill-building sessions that foster peer support and community involvement.
- Maintain a professional relationship with local Community Mental Health agencies.
- Offer immediate support and guidance during crises and follow appropriate protocols to ensure client safety and well-being.
- Participate in supervision, training, and professional development activities to enhance skills and knowledge in the field of substance use treatment.
- Bachelor's degree in social work (BSW) or related field required. Preferred experience as a Certified Peer Recovery Coach.
- Knowledge of substance use disorders, treatment modalities, and recovery principles.
- Strong communication and interpersonal skills, with the ability to establish rapport and build trust with clients.
- Ability to work effectively both independently and as part of a multidisciplinary team.
- Familiarity with community resources and social service agencies that serve individuals with substance use disorders.
- Experience providing case management or supportive services to individuals with complex needs preferred.
- Cultural competence and sensitivity to the needs of diverse populations.
- Proficiency in documentation and record-keeping, with attention to detail and confidentiality.
- Commitment to ethical practice and adherence to professional standards of conduct.
Willingness to engage in ongoing learning and professional development activities.
The duties of this position are primarily office-based, but do include travel to satellite offices and residential locations.
