LifeCircles is a Program Of All-Inclusive Care for the Elderly (PACE) that serves Muskegon, most of Ottawa and portions of Allegan Counties. The goal of LifeCircles is to provide support services to individuals 55 and older with chronic health issues to be able to age in their own community. Some of the services provided are day center attendance, home care, medical transportation to and from the day center and outside medical appointments, medical care, therapies and prescriptions. LifeCircles PACE has been providing older adults customized supports to live in their own home and avoid moving to nursing homes since 2009. By providing wrap around care that includes medical care, transportation, skilled therapies, in home care, medical equipment, home delivered meals, medications, socialization and more, seniors feel more independent, and report a better quality of life.
Driving Directions : Our Holland center is located in The Holland Town Center, just behind Brann's Steakhouse.
Business Hours : Center is open M-F 8AM -4:30 PM, our skilled medical team is available on call 24/7 to our participants and their families.
